Return Merchandise Authorization Request Form

Please follow the procedures for prompt RMA processing. Any incomplete forms will result in delays of issuing and processing of your RMA request. RMA Procedure: (Please Read Carefully) Return your defective product with our RMA form and your sales receipt - we do not issue RMA numbers. RMA's will only be accepted from the original purchaser. Clear and specific description of problem must be given for each item. ATTN: RMA must be clearly written on the shipping boxes. Shipping Damage claims must be made within 5 days. All other shipping damage claims will be refused. The items must be packaged properly. All items must be in  container with proper packaging material similar to the original packaging or the warranty will be voided. The original  signed RMA request form must be included with your return. Any damaged items (regardless of severity) will void the warranty. Credits and refunds will be based on the selling price of the product and will be charged a 15% restocking fee. 

ANY RETURN FOUND NOT TO BE DEFECTIVE WILL BE CHARGED A $15.00 HANDLING FEE THAT MUST BE PAID BEFORE THE ITEM WILL BE SHIPPED BACK. SIGNATURE NAME PRINTED DATE.
